POSITION: Responsible for facilitating access to services, managing and communicating service-related data to third parties, coordinating team communication with outside entities, and ensuring retention in treatment. This position supports clinical services by taking a lead role in actively encouraging a team approach to treatment.


QUALIFICATIONS: A Master's degree in counseling, social work, or a related field. Or a Bachelor's degree with training and experience in the mental health/drug & alcohol fields. Must have good oral, written and computer skills. Ability to work independently. Act 33, Act 34, and FBI Fingerprint Clearances required.


DUTIES/RESPONSIBILITIES:

1. Completion of required trainings: assessment, Drug & Alcohol and procedural,
2. Completion of Biopsychosocial and MHAs for Open Access, including assignment of diagnosis, level of service, and other recommendations for treatment,
3. Understanding of crisis situations requiring further assessing for hospitalization. Ability to learn and apply suicide and homicide risk assessment tools and procedures and make recommendations accordingly,
4. Backup of Intake calls when regular staff are off. Completes telephone intake calls and appropriately triages need and priorities,
5. Monitoring risk stratification report daily for determining who needs a care call, to include telephone crisis intervention, triage, and service referral,
6. Participates in all Open Access Process Improvement meetings,
7. Collaborating with support staff, HEAP team, Intake worker and Registration to facilitate smooth entry to and retention in services,
8. Process intakes from the initial call to assignment for outpatient services. Clinical review with Clinical Director and/or medical staff for case assignments if necessary,
9. May attend Treatment team meetings as needed to assist in facilitating case assignments for complex cases,

10. Responsible for sending M.H. Database to Mercer County BHC daily to inform them of all clients getting MH services,
11. Monitoring waiting list and use of counseling specialty services (including but not limited to: PCIT, EMDR, ABFT, TF-CBT).
12. Assists with medical scheduling to consider triage needs,
13. Track the OA process at the Hermitage and Greenville offices, assuring each client completes the process. (included but not limited to; registration, HEAP paperwork, WITS, assessments, assignment and connection to services),
14. Responsible for processing changes in client status using the Data Change Form/Credible as backup to Intake Caseworker,
15. Responds to requests for information both internal and external on a daily basis,
16. Entering of D&A information into the state's WITS system,

17. Work at satellite offices may be required.


· Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work.

· Adheres to agency policies and procedures/job duties or will be subject to disciplinary action.

· Performs other duties as assigned.

· Can perform the essential functions of this position with reasonable accommodations, if necessary.

· Participates in CCC CQI plan to ensure quality of service to CCC consumers.
