QUALIFICATIONS: This position functions as part of the Community Support Services team providing staff coverage. Must have good interpersonal and organizational skills in order to communicate effectively with referrals and funding sources, consumers, family members, and team members. A Bachelor's Degree in a Social Services field or a Bachelor's Degree and 2 years Human Services experience. Act 33/34 and FBI Clearances required.  CPSL certification also required.   Must maintain a current driver's license, vehicle insurance, a good driving record is part of the job site it requires transporting clients.

Initially the caseworker will meet with the participant a minimum of two (2) hours per week to create the participant's recovery plan and then discuss and work on recovery goals. 

Supportive Housing Services are designed to help persons with Opioid Use Disorder transition into their own homes.  As such, the worker will need to understand principles of drug and alcohol recovery and work closely with drug and alcohol counselors working with the client as well as be able to network and work with other agencies and individuals serving the clients such as case manager, probation/parole officers, etc.  Training will be provided to augment skills with working with this population of clients.

After the client has moved into independent living, staff will work with them to remain stable in the community.  Staff can assist with all aspects of daily living.   


  1. Will be responsible for a caseload of 9-13 clients living in Mercer County.
  2. Will assist with all aspects of daily living to include but not limited to:
  3. a)  Making recovery-oriented appointments, such as drug and alcohol treatment, 12 step     programming, etc., help coordinate follow-up care to meet client needs
  4. b)  Basic communication skills-such as using a regular phone, mobile phone, email or the Internet
  5. c)  Transportation - either by driving oneself, arranging rides or the ability to use public transportation
  6. d)  Meal preparation - meal planning, preparation, storage, and the ability to safely use kitchen equipment
  7. e)  Shopping- the ability to make appropriate food and clothing purchase decisions
  8. f)  Housework - doing laundry, cleaning dishes and maintaining a hygienic place of residence
  9. g) Managing medications - taking accurate dosages at the appropriate times, managing re-fills and avoiding medication interactions
  10. h) Managing personal finances- operating within a budget, writing checks, and paying bills
  11. i)  Finding and securing housing-getting utilities started, repairs completed, obtaining furnishings and basic household items purchased, or received
  12. j) Learn to be a good neighbor and how to understand and follow lease/rental agreements
  13. k) Utilization of community resources to build a natural support system
  14. Complete needed information in the credible system to open and close a consumer to services.
  15. Complete initial assessments, reviews, progress notes and all other contacts on behalf of client as needed in the credible system.
  16. Completes authorizations and intakes in a timely manner to facilitate the billing through Value.
  17. Meets with each person on the caseload monthly (some will be weekly/twice per week- depending on need) to discuss and document process toward goals.
  18. Documents monthly a summary of progress for each client on the case load in credible.
  19. Insures that after each encounter with a client, that the client's signature is captured on the note in credible.
  20. Functions as an effective team member in the development of service/recovery plans for the clients.
  21. Serves as an effective role model for good work habits, good grooming, appropriate social skills and exhibits professional demeanor at all times.
  22. Networks and maintains positive working relationships with those involved in the client's life; such as other Mental Health professionals, human services workers, probation/parole, employers, family members, and significant others etc. in order to provide effective services.
  23. Maintain a database of the clients in the programs. This will include monitoring of provision of hours/units for each client that is compliance with productivity standards set by the agency.
  24. Develops creative ways to challenge clients to stay involved, increase work skills, improve work habits, and socialize in a way appropriate to the community.
  25. Participates in activities on-site and in the community in a professional manner.
  26. Attends weekly staff meeting with supervisor and peers.
  27. Will complete weekly schedule in credible the Friday prior to the beginning of the next week.
  28. Will adhere to the schedule submitted Friday or will notify supervisor of the changes.
  29. Attends relevant trainings, workshops, as assigned to advance skills.
  30. Is knowledgeable about the safety procedures on and off site.
  31. Provides at least 80% of direct service each week.
  32. Participates in monthly meetings to discuss cases and strategies that are relevant to the caseload.
  33. Maintains and upholds Community Counseling Center's policies, respects confidentiality of clients.
  34. Performs other duties when assigned.

Supervision is received from the Rehabilitation Manager who will assign work based on client need. Employee's work will be reviewed through observation, electronic health record data input and content and timely submission of work involved with the program.
